Calcium Supplements: The Honest Pros and Cons

The short answer

Adults need 1,000 to 1,200 mg of calcium a day depending on age and sex, and most people get closer to that from food than they assume. Supplements carry small but real downsides: a contested cardiovascular signal, more kidney stones in the largest trial, constipation, and interference with common medications. Count what you already eat first. A supplement is for closing a measured gap, not for insurance.

"I've taken calcium every morning since my mother broke her hip. Should I still be?" Some version of that question comes up here most weeks, almost always from a woman who has taken the same supplement for a decade, has never been told a target number, and has read a headline about calcium and heart attacks that she has been quietly worrying about ever since.

The honest answer starts somewhere unexpected: not with the supplement, but with breakfast. Once you count what you already eat, the supplement question usually answers itself.

How much you actually need, and how close you already are

The recommended dietary allowances come from the National Academies, published by the NIH Office of Dietary Supplements. They are simpler than most people expect:

  • Adults 19 to 50: 1,000 mg a day, men and women.
  • Men 51 to 70: 1,000 mg a day. Women 51 and older: 1,200 mg a day.
  • Everyone 71 and older: 1,200 mg a day.
  • Tolerable upper intake level for adults over 50: 2,000 mg a day from food and supplements combined.

Now the part that changes the conversation. Average daily calcium intake in the United States from food and beverages alone is 1,083 mg for men aged 20 and older and 842 mg for women. The average man is already at his target before he swallows anything. The average woman over 50 is roughly 350 mg short, which is one container of yogurt or one glass of fortified orange juice. That gap is worth closing. It does not require a 1,200 mg tablet, and taking one on top of an already adequate diet is how people drift toward the upper limit without intending to.

Food first, with the numbers that make it decidable

We push food first for a specific reason, not sentimentality. Dietary calcium arrives slowly, in modest amounts, alongside protein and other minerals, and it does not carry the two problems that follow supplements: the cardiovascular question and the kidney stone question. Neither signal has ever attached to calcium from food. Here is what common servings deliver, per the NIH Office of Dietary Supplements. Add up a normal day of yours.

  • Plain low-fat yogurt, 8 oz: 415 mg
  • Fortified orange juice, 1 cup: 349 mg
  • Part-skim mozzarella, 1.5 oz: 333 mg
  • Canned sardines with bones, 3 oz: 325 mg
  • Nonfat milk, 1 cup: 299 mg, and fortified soymilk, 1 cup: 299 mg
  • Firm tofu set with calcium sulfate, half cup: 253 mg
  • Canned salmon with bones, 3 oz: 181 mg
  • Cooked kale, 1 cup: 94 mg, and chia seeds, 1 tablespoon: 76 mg

Two of the first five items put you most of the way to 1,000 mg. Almost nobody does this arithmetic before buying a supplement, and it turns a vague worry into a number you can act on.

The honest case against supplements

Most articles either wave these concerns away or overstate them. The literature is contested, so here is what it says.

The cardiovascular question. In 2010, Bolland and colleagues pooled 15 randomized placebo-controlled trials in the BMJ and found more myocardial infarctions in people assigned calcium: a hazard ratio of 1.31 in the patient-level analysis, a relative risk of 1.27 in the trial-level one. A 2011 reanalysis of the Women's Health Initiative pointed the same way. Then the evidence split. A 2016 systematic review in Annals of Internal Medicine found no significant difference in the randomized trials, and that same year the National Osteoporosis Foundation and the American Society for Preventive Cardiology issued a joint guideline concluding there was moderate-quality evidence of no relationship, with total intake up to 2,000 to 2,500 mg a day safe from a cardiovascular standpoint. In 2021, a meta-analysis in Nutrients restricted to double-blind trials found the signal again, roughly 15% higher risk, concentrated in healthy postmenopausal women.

The honest conclusion: the signal is inconsistent, small if real, and confined to supplements. The randomized evidence that produced it tested supplements, and the 2016 review found no consistent dose-response relationship between dietary calcium and cardiovascular risk. That is not a reason to panic about a supplement you need. It is a good reason not to take one you do not.

Kidney stones, where the intuition runs backwards. In the Women's Health Initiative trial, 36,282 postmenopausal women took 1,000 mg of calcium carbonate with 400 IU of vitamin D daily or placebo for an average of seven years. Urinary tract stones were more common in the supplement group, hazard ratio 1.17. Dietary calcium does the opposite: in a prospective study of more than 45,000 men in the New England Journal of Medicine in 1993, Curhan and colleagues found the highest quintile of dietary intake carried about half the stone risk of the lowest. Calcium eaten with a meal binds oxalate in the gut so it never reaches the urine; a supplement swallowed between meals does not get that chance. If you have a stone history, "avoid calcium" is the wrong instruction.

Gastrointestinal effects. Constipation, bloating and gas are the most common reasons people quietly stop taking calcium, and they are more frequent with carbonate than citrate. A reason to switch forms, not to abandon a supplement you need.

Drug interactions, which patients are rarely warned about. Calcium binds several common medications in the gut:

  • Levothyroxine. Keep calcium at least four hours from your thyroid pill. This is a common reason a stable thyroid dose quietly stops working.
  • Quinolone antibiotics such as ciprofloxacin and levofloxacin need at least two hours of separation on either side. Tetracyclines have the same problem.
  • Iron. Calcium reduces iron absorption when the two are taken together, so separate them.
  • Acid suppression. Calcium carbonate needs stomach acid to dissolve. If you take a proton pump inhibitor, carbonate is the wrong choice and citrate is the right one.
Worth saying plainly

A calcium supplement is not a bone protection plan. It is a way to close a gap you have actually measured. If your diet already lands near your target, adding a tablet takes on the downsides without buying you much, and no amount of calcium substitutes for weight-bearing exercise, adequate protein, and treating osteoporosis when you have it.

If you do supplement: form, dose, and vitamin D

Three rules cover almost everything. First, absorption is best in doses of 500 mg or less, so split a supplemental target across the day rather than taking one large tablet. Second, carbonate with food, citrate any time: carbonate is the most concentrated form but depends on stomach acid, while citrate is gentler and works without it, which makes citrate the right choice on acid-suppressing medication. Third, vitamin D belongs in the conversation, because it is what allows calcium to be absorbed at all. The recommended intake is 600 IU a day through age 70 and 800 IU from 71.

One honest note on the evidence. The US Preventive Services Task Force, in its 2018 final recommendation, recommends against daily supplementation with 400 IU or less of vitamin D plus 1,000 mg or less of calcium for primary fracture prevention in community-dwelling postmenopausal women, and calls the evidence insufficient at other doses. That statement is under review and does not apply to people with osteoporosis, a prior fragility fracture, or documented deficiency. The corrective stands: routine supplementation in healthy people has never shown the fracture benefit the supplement aisle implies.

Calcium carbonateThe most calcium per tablet and the least expensive. Take it with food, split doses at 500 mg or less, expect more constipation. Poor choice on acid-suppressing medication.
Calcium citrateLess elemental calcium per pill, so more pills, but absorbed without stomach acid and better tolerated. The right default for anyone on a proton pump inhibitor or with a sensitive gut.
Vitamin D alongsideWorth getting right, because calcium absorption depends on it. Dose should follow your measured level rather than a marketing number, and more is not better.
Coral calciumNo advantage over ordinary calcium carbonate. Its marketing was the subject of a Federal Trade Commission action against Kevin Trudeau beginning in 2003 over claims that it could cure cancer and heart disease, ending in a 2004 settlement banning him from infomercials. We do not recommend it.
"Bone support" blendsCalcium packaged with boron, strontium, silica or proprietary complexes and sold on mechanism rather than outcome. None of these blends has fracture-reduction evidence behind it, and the added ingredients complicate the interaction list.
Mega-dose single tabletsA 1,200 mg tablet exceeds the amount you can absorb at once, so you pay for calcium you excrete while pushing your total toward the 2,000 mg upper limit. There is no situation in which this is the better product.

How this works at Framework Health

Questions we hear about this every week

I've taken calcium for years. Should I stop?
Not automatically, and not on your own if you have osteoporosis or a fracture history. Bring the bottle to your next visit and add up a typical day of food first. If your diet already delivers close to your target, the supplement is doing more for the manufacturer than for your bones.
Do calcium supplements cause heart attacks?
The evidence is genuinely mixed. A 2010 BMJ meta-analysis and a 2021 meta-analysis of double-blind trials both found a modest increase in cardiovascular events, while a 2016 Annals of Internal Medicine review and a joint National Osteoporosis Foundation and American Society for Preventive Cardiology guideline found no relationship and called intakes up to 2,000 to 2,500 mg a day safe. It has never appeared for calcium from food.
I get kidney stones. Should I avoid calcium?
No, and this is the point most often gotten backwards. Restricting dietary calcium increases oxalate absorption and makes stones more likely. Prospective data show higher dietary calcium intake is associated with roughly half the stone risk. Supplements are the form associated with more stones, and taking them with meals rather than between them reduces that risk.
Does calcium interfere with my thyroid medication?
Yes, meaningfully. Calcium binds levothyroxine in the gut and reduces how much you absorb, which can quietly destabilize a dose that had been working for years. Keep the two at least four hours apart, and tell whoever manages your thyroid that you take calcium.
I don't eat dairy. Can I still hit the target from food?
Comfortably, yes. Fortified orange juice and fortified soymilk deliver about 299 to 349 mg per cup, calcium-set tofu about 253 mg per half cup, and canned sardines or salmon eaten with the bones add 181 to 325 mg per three ounces. Two or three of those a day gets most people there without a tablet.

The bottom line

Most adults should get their calcium from food, and most are closer to the target than they think. Supplements exist to close a gap you have measured, at the smallest dose that closes it, in the form your stomach and medication list can handle. If you have osteoporosis, a prior fracture, or a condition that impairs absorption, the calculation changes and supplementation is often clearly warranted. What is not warranted is a decade of tablets taken as insurance against a number nobody ever told you. Bring the bottle in and we will do the arithmetic with you.
